Provides a wide variety of community health programs, with services including: Asthma Management, Chronic Disease Care, Diabetes Management, Doctor or clinic, Flu Shot, Urgent Care Notes: Confidential primary care services include urgent care, primary care referrals, general health education, blood pressure and blood glucose monitoring, medication review, wound and infection treatment, flu shots, cold and virus treatment, and referrals for more in-depth treatment. The care team also provides some case management and referrals to support services. Services are for homeless persons.
  • HoursFri 9am-4pm (primary care clinic) and third Wed of the month 9am-11:30am (podiatry only)
  • Specific Populations Served:0-5, 5-11, 11-14, 14-19, 24-59, 60+, 19-24
  • Fees:All services are free.
  • Application Process:This is a drop in clinic only. The clinic at St. Vincent de Paul's is for the homeless population.
